Abstract
This study sought to determine the risk factors associated with Preterm Premature Rupture of Membranes (PPROM) and outcomes of deliveries complicated by Preterm Premature Rupture of Membranes (PPROM) in Mbarara Regional Referral Hospital (MRRH). A total of 274 mothers were recruited in a prospective un- matched case control study in a ratio of 1 case to 3 controls to determine the risk factors associated with PPROM, at MRRH. Social demographic variables, obstetric history and information on medical factors were collected. High vaginal swabs were collected and analyzed for bacterial colonization. Univariate and mmultivariate analysis to determine associated factors with PPROM was done. A p –value of 0.05 was considered statistically significant. Primary outcomes included alcohol intake during pregnancy P=0.01 (OR) 3.8 1.3-11.2(95%CI, HIV infection) P=0.03 (OR) 2.5 1.1-5.6(95%CI, presence of symptomatic urinary tract infection P=0.02 (OR)2.6 1.2-5.9 (95%CI), short inter pregnancy interval P=0.01 (OR)0.2 0.1-0.5(95%CI), high parity greater than 4 P=0.01 (OR)14.9 4-54.9(95%CI) and poor antenatal attendance less than 3 P=0.03 (OR) 2.5 O.1-5.6 (95%CI) were significantly associated with preterm premature rapture membranes. Secondary outcomes of PPROM were associated with poor pregnancy outcomes like prematurity; gram negative organisms were isolated from the high virginal swab cultures. The study concluded that risk factors associated with PPROM included, alcohol intake during pregnancy, presence of urinary tract infection and having a short inter pregnancy period and a poor antenatal care clinic attendance during the pregnancy and high parity
