Abstract
The study investigated fiscal decentralization and health- sanitation service delivery in Makindye division, Kampala district. It was guided by the following objectives to examine the contribution of fiscal decentralization towards the health infrastructure in Makindye Division, to analyze the contribution of fiscal decentralization over health workers in Makindye Division and to determine the contribution fiscal decentralization has on the availability of health services and facilities in Makindye Division. The study adopted a cross-sectional study design, the researcher purposely selected 71 respondents consisting of municipal health officers, the chairpersons, LC III, the sub-county chiefs, Community Development Officer, the mayor, health workers and members of the community. The sample size was selected using the table by Krejecie and Morgan cited by Amin, (2005) thus from the selected study population of 71 a sample size of 60 was obtained the study found out that that the contribution of fiscal Decentralization on the Health-Sanitation infrastructure was rated at different levels. Out of the items, two (2) were rated very high. equivalent to strongly agree meaning that respondents strongly agreed with the statement. The remaining eight (8) item were rated high (equivalent to agree) meaning that the respondents agreed to the statement, the contribution of fiscal decentralization over Health workers was rated at different levels. Out of the items, two (2) were rated very high equivalent to strongly agree meaning that respondents strongly agreed with the statement. The remaining eight (8) item were rated high (equivalent to agree) meaning that the respondents agreed to the statement and the contribution fiscal decentralization on the availability of health services and facilities were rated at different levels. Out of the items, six (6) were rated high equivalent to agree meaning that respondents agreed with the statement. The remaining four (4) item were low (equivalent to disagree) meaning that the respondents disagreed to the statement with. The study concluded that Fiscal decentralization policy has led to increased utilization of the Health facilities with the perception that the policy was good because it empowered the local community in terms of creating sense of ownership in the stakeholders, that has facilitated the sustainability of the public institution .in spite the views expressed, the policy has failed to deal with drug shortages, inefficient utilization of the resources ,morale among the staff, retraining of the staff and better remuneration in order match effective implementation of the policy. The study recommended that it is important that national governments and UNDP officers involved in fiscal decentralization reforms bear in mind the positive effects, as well as the risks connected to fiscal decentralization, in order to maximize the benefits and avoid negative side effects through appropriate designing and implementing of fiscal decentralization reform. Makindye Division, must ensure that they hold continuous trainings on educating the locals on what exactly is the meaning of Fiscal decentralization since many people conceptualized fiscal decentralization depending on their culture. Donor involvement in the availability of the conditional grants for Health-Sanitation, roads and education and closer monitoring by the central government, donors and the non-governmental organization and Effective coordination and monitoring by the central government, local governments can be strengthened so that Health-Sanitation service policies are effectively implemented.
